The CaNSuN Resource Guide
Patient Navigators and Community Health Workers help patients get around a variety of barriers. The CaNSuN Resource Guide is a downloadable excel spreadsheet that provides an overview and links to resources to help patients overcome common barriers, including lack of transportation, tight finances, and lack of social support. Click the link above to open and download the guide to make it your own – highlight resources available in your area or ones you use most frequently and add more that you use but might not already be included. Please note that resources linked in the Guide are primarily geared toward Pennsylvania and the area surrounding the Penn State Cancer Institute, with a few exceptions. The CaNSuN resource guide based upon a patient resource guide created by the George Washington University. This guide, while specific to the Washington, D.C. area, has many links to nationwide resources that might be helpful.
The George Washington University Cancer Institute also has developed a Patient Navigation Barriers Barriers and Outcomes Tool (PN-BOT), which is a free Excel-based data entry, data management, and reporting product that is designed for oncology patient navigation programs. It can be used to document, track, and generate reports on data including patient volume and demographics, cancer treatment profiles and timeliness of care, barriers to care, patient outcomes, and others.
